By registering for this program, I AGREE & ATTEST to all the following, on behalf of myself named herein as a participant in this program or any minor participant (s) mentioned herein that I represent and am legally responsible for.

 

I AGREE on behalf of myself as the participant or participants named herein to agree to hold harmless, release, waive and indemnify Artfully Crafty, its members, employees, agents, successors and assigns to any & all injuries, illness, medical conditions, claims, liabilities, damages, expenses or judgments from participation in any and all Artfully Crafty’s programming and/or activities, including use of the area(s) and/or equipment, participation in a class or program at Artfully Crafty. 

 

Any defense, cost or expense arising out of any and all claims, injuries, liabilities or damages arising out of or related to participation in any and all of our programming, as set forth, including attorneys’ fees. 

 

I AGREE NOT TO ATTEND this program if I as a participant or any minor participant(s) mentioned herein, or any member(s) of my household, have been diagnosed with COVID-19 within 14 days of this program. 

 

I CERTIFY the information that I have entered herein to be accurate and true. I am at least 18 years old and I have read and agree to the terms of this agreement. 

 

I CERTIFY that I am the parent or legal guardian of the above-mentioned minor participant(s) mentioned herein & confirm that the information I have entered here is accurate and true. I CERTIFY that I am responsible for any caregiver or guardian mentioned herein and guarantee that all of the above is true and correct. I CERTIFY that I am at least 18 years old and I have read and agree to the terms of the above agreement. 

 

MATERIALS: For classes, all materials are SUPPLIED unless otherwise communicated. ART SMOCKS are not included. If your child wishes to wear an art smock, please bring it to the class with you. 

 

SNACKS, DRINKS, BELONGINGS (required)

Please DO NOT share any belongings, food, or drinks with any other student or staff member. Please do not bring andy non-essential items to our studio. This includes bags, bicycles, toys, stuffed animals.

Artfully Crafty may take photographs or videos of artwork or work in progress at the studio during classes. These photos may be published on our social media pages or platforms &/or website, &/or may be posted on other promotional materials or in other promotional media. NO STUDENT will be identified in these photographs or tagged/identified in any photo without their consent. Please note, we archive all work created at our studio. If you do not wish your child to be in a group photo, please state this clearly in the notes section of this form. All photographs are the property of Artfully Crafty’s and we do not allow these photos to be published by third-party or by any other media or source. We value & are protective of our students, their interests & privacy.

CLASS START & END TIMES

All students are expected to arrive on time & picked up promptly unless communicated previously. Classes start & end at the advertised times. If a student arrives late, they will NOT be given extra time at the end of class to finish their project. If a child is picked up late and this has not been previously communicated, and extra charge can be added at the discretion of Artfully Crafty. 

 

RESCHEDULING

Cancellation + Rescheduling Policy: To reschedule a class, please give at least 24 hours in advance. If you fail to notify us in time (at least 24hrs), you are not entitled to any make-ups, rescheduling, credit, or refunds.

Please list anything we should be aware of, or any special requests, allergies pertaining to the attendee so we can make your experience at our studio a joyous one.
